お名前とご所属、免税措置希望有無を記載の上、下記、スポンサー事務局宛にメールにてご連絡ください。返信の形で、申込書をお送りいたします。
※税制上の優遇措置が必要な場合は、事務局口座とは異なる口座へのお振込となります。
